All femoral hernias should be repaired as an urgent elective procedure, as they are particularly likely to obstruct and strangulate.


There are three classical approaches:

  • Low (Lockwood)

  • Transinguinal (Lotheissen)

  • High (McEvedy)


Irrespective of approach, the aims are to:

  • Excise the sac.

  • Reduce the contents.

  • Obliterate the femoral ring by suturing the inguinal ligament to the pectineal ligament.
